Prior to working in certain conditions, a medical surveillance must take place. Medical surveillance is the systematic assessment of employees exposed or potentially exposed to occupational hazards. 

This assessment monitors individuals for adverse health effects and determines the effectiveness of exposure prevention strategies.

As the employer you must complete the Annexure 3 document prior to your employees going to the medicals. The completed Annexure 3 as well as the employees ID copy must be taken with to the medical appointment.

Add the medical certificate to the safety file once it has been issued to you.
